Wound Bed Preparation: An evidence-based approach

There is a critical need for evidence to support efficacy in wound closure with minimal impact on patients, particularly for Wound Bed Preparation.*

According to EWMA and WHO guidelines, Wound Bed Preparation aims to remove local barriers to healing promoting wound closure through diagnosis, appropriate treatment of the cause and attention to patient-centered concerns.*

Making the ideal cleanser choice

Wound cleansers have been historically used solely to remove contaminants, foreign debris and exudate.

Today, wound cleansing has become recognised as a fundamental step of Wound Bed Preparation with the potential to promote healing by removing local barriers to healing,* while at the same time minimising the harm to wound tissue.*

The cleanser solutions available on the market have several limitations and traditional cleansing agents have not been shown to have efficacy in wound care.*

How Nexodyn fits in – An active wound cleanser

Adding Nexodyn® AOS to the standard of care for diabetic foot ulcers results in faster healing time and protection from risk of infection.*

  1. Healing time was significantly reduced in the group receiving Nexodyn® AOS*
  2. Significant protection from risk of infection*

Nexodyn AOS reduces clinical signs of local infection and wound associated pain in leg ulcers.*

  1. Significant reduction of clinical signs of local infection*
  2. Wound-associated pain was significantly reduced over time*

Nexodyn® AOS is well tolerated No adverse events associated with the use of Nexodyn® AOS were reported in any of the reported studies.*

A wealth of evidence and real-world experience consistently show that Nexodyn® AOS reduces healing time, protects from risk of infection, reduces clinical signs of local infection and wound associated pain with a highly tolerable profile.*

What is TEHCLO™: THE NANOTECHNOLOGY BEHIND NEXODYN® AOS*

Tehclo™ is the APR proprietary global patented nanotechnology platform that applies nanoscience to the production of a unique hypochlorous acid (HClO) solution. Hypochlorous acid is well-known as a broad-spectrum, fast acting antimicrobial agent naturally produced by neutrophils as part of the innate immune system’s response to infections.

Unique combination of 3 physicochemical properties:

  • High purity – >95% Hypochlorous Acid (HCLO), 0% Ion Hypochlorite (CLO) and < 5% Chlorine (CL2)
  • Low pH – 2.5 – 3.0
  • High ORP – Oxidation Reduction Potential 1,000-1,200 mV

INDICATIONS FOR USE: The device is an AcidOxidizing Solution (AOS) intended for use in the debridement, irrigation, cleansing and moistening of acute and chronic wounds (e.g. diabetic foot ulcers, pressure ulcers, lower leg ulcers, vascular ulcers), post-surgical wounds, cuts, abrasions, burns and other lesions. Nexodyn AcidOxidizing Solution (AOS) contains hypochlorous acid (HClO), a known antimicrobial agent acting as a preservative by inhibiting the growth of microorganisms within the solution, and preventing wound contamination by exerting a local antimicrobial effect on the wound surface. Thanks to its physico-chemical properties and by reducing the burden due to presence of microorganisms, this solution creates an ideal microenvironment to sustain the physiological healing process, especially relevant with chronic wounds, favouring an optimised lesion closure.

HOW TO USE: At each medication, the whole lesion area should be thoroughly moistened with Nexodyn AcidOxidizing Solution (AOS) and allowed to dry a couple of minutes. A second application could be consecutively repeated, if needed. When the Nexodyn AcidOxidizing Solution (AOS) has dried, other procedures can be performed on the lesion, based on the recommended clinical practice. At the discretion of the healthcare professional, treatment with Nexodyn AcidOxidizing Solution (AOS) can be performed and/ or prescribed up to 2 times per day.

MECHANISM OF ACTION: The device is a wound irrigation, cleansing, and moistening solution, with ancillary antimicrobial activity. The mechanism of action is based on the mechanical action of the fluid flowing through the lesion and removing biologic and inert materials such as microorganisms, biological debris and environmental dirt.

Treatment with Nexodyn AcidOxidizing Solution (AOS) creates an ideal microenvironment to sustain the physiological healing process thanks to three features the device: highly pure HClO, pH between 2.5 and 3.0 and high Reduction-Oxidation Potential.

Hypochlorous acid exerts an ancillary antimicrobial action on the wound surface, enhancing the reduction of the local bacterial load. HClO, characterized by a neutral charge, efficiently penetrates the negatively charged bacterial membrane, thereby exerting its antimicrobial action.

pH ≤ 3.0 reduces bacterial growth by creating an unfavourable environment. Furthermore, the acid environment promotes tissue oxygenation by the Bohr effect, and inhibits the penetration into healthy cells of ammonia produced by bacteria catabolism.

The high Reduction-Oxidation Potential physically damages the cellular membrane of micro-organisms, inactivating their defence mechanisms.

The combination of low pH and high Reduction-Oxidation Potential has a role in the inhibition of matrix metalloproteases (MMPs), known to have an important role in the promotion of inflammatory processes and in the chronicisation of wounds. The inhibition of MMP activity in chronic wounds has been shown to contribute to interrupt the vicious circle of delayed wound healing, thus promoting the restart of the physiological healing process.

HANDLING: The device is non-irritating to human skin, eyes or mucosa, and is not a skin sensitizer. Special handling precautions are not required.

STORAGE: The device should be stored in its original sealed container between 5°C and 25°C. It is non-flammable. Special storage precautions are not needed. After use, the container should be closed again, before storing.

STABILITY: The device maintains its microbiological characteristics during its shelflife; each batch is controlled by sterility tests according to the current edition of the European Pharmacopoeia for the verification of the efficacy of preservative agents in preparations for topical use.

Until it is opened, the stability of the device is ensured up to expiration date (see bottle for year and month of expiration). Upon opening, please note the date of opening in the box “Date Opened” on the bottle. Stability of the unused solution is ensured up to 30 days after first opening.

DIRECTIONS FOR USE:

Nexodyn AcidOxidizing Solution (AOS) is ready for use with no mixing or dilution required. Apply Nexodyn AcidOxidizing Solution (AOS) directly on the affected area.

Directions for use with spray cap (for 100 ml and 200 ml only):

• Open the bottle by unscrewing the cap.

• Open the plastic sachet containing the spray cap.

• Take the spray cap from the sachet from top (cap), taking care not to touch the straw.

• Insert the straw into the bottle.

• Screw the spray cap on the bottle neck.

• Before using the product for the first time, spray in the air to start the pump, until the solution is sprayed.

• Spray Nexodyn AcidOxidizing Solution (AOS) directly on the injured zone, until the affected area is thoroughly covered.

• Do not unscrew the spray cap after its installation; do not screw the original cap back onto the bottle after the installation of the spray cap.

• The device can be used for up to 30 days after first opening.

Directions for use without spray cap (for both 500 ml and 250 ml):

• Open the bottle by unscrewing the cap.

• Apply Nexodyn AcidOxidizing Solution (AOS) directly on the lesion. The solution can be poured directly, or, alternatively, it can be sucked up into a sterile syringe (without needle) immediately before use and then applied, in order to thoroughly cover the affected area. In the latter case, always use sterile syringes, CE marked as Medical Devices.

• Close the bottle with the screw cap immediately after use.

• The device can be used for up to 30 days after first opening.
